Honestly, judging by the fact of how strict Nintendo were with the stuff that could and could not go into their games, that's not a group effort. Maybe one person did it who was like "Ah, fuck it, if it passes QA, I'll be the jackass who gave toad two middle fingers. Sweet!". If management knew of this at the time, they would have crucified whoever wrote it in. Of course, we are talking about 8-bit technology and this could have just been an oversight on someones behalf. Funny how no one, until now, realized that was even the case.
